A Just Culture speaks up for patient safety

A culture of change is arriving at UPMC. It is an approach to patient safety unlike any other, and it’s called “A Just Culture.” A Just Culture will empower our physicians, nurses, and other health care providers to voice concerns about patient safety without fear of retaliation, and will ensure confidence that leaders will listen and take action in a fair and consistent manner.

A concerted effort will be undertaken to increase physician use of electronic order sets that can be accessed from eRecord, UPMC’s electronic medical record.

Electronic process to be implemented for surgeons to validate pathology findings

At many UPMC hospitals, the surgeon is required to validate a patient’s pathology findings as being consistent with the surgeon’s clinical diagnosis. To improve efficiency and timely completion of this documentation, a new electronic process will be implemented to obtain pathology validation from surgeons.

To address this challenge, and to start conversations around topics involving dialysis and renal care, dialysis unit staff members began wearing scrub tops designed as walking billboards.

Death certificates revised for start of new year

Beginning Jan. 1, 2012, the Department of Vital Records is implementing a revised Certificate of Death that will replace the white death certificate currently used for all neonates and adults.
